  • Filtered to the F-tags you pick: infection control, falls, dietary and more
  • Facility, city, F-tag, severity and survey date, with a link to the CMS record

How it works

From CMS inspection to a sales conversation

CMS publishes survey results

State surveyors inspect Medicare and Medicaid nursing homes. CMS publishes every health citation about once a month.

We find what's new for you

We compare each release with the last one and keep only new citations in your state, for the F-tags you chose.

You reach out first

You get an email with each newly cited facility, the F-tag, the severity and a link to the public CMS record.

Who it's for

LTC and compliance consultants

Offer plan-of-correction, mock-survey and QAPI help to facilities cited for the tags you specialise in.

Staffing agencies

Citations for care quality and supervision often point to staffing gaps. Reach those facilities early.

Training and education vendors

Infection control, dietary and abuse-prevention citations often lead to in-service training.

The most-cited F-tags, and who sells help with each

Citation and facility counts are from CMS surveys Oct 2025–Sep 2026. The last column is our view of who typically offers help with each finding, not a CMS category.

Why pay instead of reading CMS data yourself?

The CMS file has every citation from the last three survey cycles in one large table. Finding what is new since last month, for your state and your F-tags, means downloading, comparing and filtering each release. We do that and send you the short list.

Get the facilities newly cited in your state

We're considering an email alert: each time CMS publishes new inspection results, a list of the nursing homes in your state newly cited for the F-tags you pick, $49/mo per state. Built for LTC consultants, staffing agencies and training vendors. It isn't available yet and nothing is charged. Join the waitlist and we'll email you if it launches.

Data you can check

Built on public CMS inspection data

Every citation comes from the CMS Provider Data Catalog Health Deficiencies file, which lists findings from state surveys of Medicare and Medicaid nursing homes. We count and sort; nothing is estimated.

  • Standard and complaint health surveys
  • Scope and severity as published by CMS
  • A citation is a finding; facilities may dispute it

Questions

Where does the data come from?

From the CMS Provider Data Catalog Health Deficiencies dataset (released 2026-10-28), which covers health citations from state surveys of Medicare and Medicaid nursing homes.

How fresh are the citations?

CMS updates the file about monthly. The current file covers surveys from 2025-10-01 to 2026-09-30; the next update is expected around 2026-12-02.

Is the alert available today?

Not yet. Join the waitlist and we'll email you if it launches. Nothing is charged and no card is collected.

Does a citation mean a facility did something wrong?

A citation is a survey finding as published by CMS. Facilities can dispute findings, and severity ranges from potential to actual harm. Treat it as a reason to start a conversation, not a judgement.
